New Unit owners can use this page in lieu of the paper in-service card.
Secondary owners can use this form to notify Altec of a change in ownership. This notification is part of the ANSI standard under Responsibilities of Owners – Transfer of Ownership.
The ANSI Standards states “When a change in ownership of a digger derrick or aerial device occurs, it shall be the responsibility of the seller to provide the manufacturer’s manual(s) for the digger derrick or aerial device to the purchaser. It is the responsibility of the purchaser to notify the manufacturer of the unit model and serial number and the name and address of the new owner within 60 days.”
Please complete the information below. This will allow Altec to update the customer mailing list so that necessary manufacturer/user communications can be sent to the new owner.
Altec Worldwide Distributors: Complete this form with the end-user’s information, not your company information. Please note that the warranty period begins the date your company delivers the unit to the customer/end-user or the date your sales associate or Altec sales associate puts the unit in-service. This is the date that must be entered in the “In-Service Date” field below.
